The below fees apply to clients receiving Home Care Package from the Australian Government.

Care Management ( Fully Managed)

High-quality service by specialised team working to improve your well-being through care planning and education. You will be allocated a dedicated Care Coordinator. Capped at 10% of quarterly budget.
